Intake & Referrals

Referral Process for Children's RCS:

Guardians or Case Managers can access services by submitting an application for eligibility directly to www.qualitycareforme.com .

Referral process for Case management & Outpatient services:

• Referrals can be submitted directly by the client seeking services or by case managers.

• Upon receiving your referral form, we will contact you, the client to complete a prescreening over the phone.

• We will answer any questions you have and with your consent proceed to complete an intake with full assessment.

• We will then connect you with a case manager/mental health professional who will work with you to explore your goals and create your individualized treatment/ service plan.

Referral process for Personal Care services:

For Mainecare recipients, please contact directly your assigned Care Coordinator from any of the Service Coordination Agencies (i.e. Alpha One, SeniorPlus or Care Lync) to select services with our agency.
